Monitoring and Evaluating a Health Statewide Initiative

NSW Health and the NSW primary health networks (PHNs) have developed a statewide initiative for Diabetes Management, under a ‘one health system approach’ to improve the health outcomes and experience of care for people living with diabetes or at risk of developing secondary complications.  The initiative addresses key opportunities to build on existing diabetes management activities of local health districts (LHDs), specialty health networks (SHNs), primary health networks (PHNs) including general practice, and other primary and community care organisations.

Under this more coordinated approach, people living with diabetes should be able to access and receive care that is appropriate to their needs and preferences and supports them to achieve health outcomes that matter to them, is provided in a timely, planned, coordinated and integrated manner and is delivered by a team of health practitioners from primary, community, aged care and hospital settings.

Nexus has been commissioned to develop a framework to monitor and evaluate the implementation of the statewide initiative. The plan will incorporate qualitative and qualitative measures across the four domains of Value Based Healthcare – patient outcomes, patient experience of care, clinician experience of care and sustainability/efficiency.

This framework will be informed by a series of workshops with key stakeholders, including, clinicians to develop a program logic, key evaluation questions, and potential data sources.
